Membership Application Terms and Conditions
Consent by Parent / Guardian
1. I give my consent for my child to take part in the activities of Woking Blackhawks Basketball Club and acknowledge that he/she undertakes basketball and associated activities at my/their own risk and responsibility.
2. I give my consent for the administration of basic first aid treatment by Coaches or Team Managers or to be taken to hospital in an emergency.
3. I give my consent for the administration of emergency medical treatment to my child in the event I cannot be contacted.
4. Pictures of my child engaged in Club activities may be made available by the Club to the local press or published on the Club's or related sports internet sites with the sole intention of promoting youth sport and the interests of the Club. Please contact us if you wish the player to be excluded.
5. We (Woking Blackhawks Basketball Club) use your child's personal data to manage and administer their membership in the Club. We use your personal data to send you emails or text messages regarding news and events that relate to your child's basketball activities within the Club, on the understanding that you may be interested in their content. Accordingly, we have a legitimate interest for holding your and your child's personal data.
6. We rely on your consent for any sensitive data you share with us, such as health data. You may withdraw your consent at any time.
7. Some data is shared with Basketball England, Wessex Basketball Association and Surrey Basketball Association, as appropriate, who use the data to regulate, develop and manage the game. We will not use any personal data you have provided to us for any other purpose, nor will we share it with any other organisation. You can opt out of our mailing lists or social media groups at any time by emailing your team manager or organiser.
8. You can also ask us to access, rectify or erase the personal data we have about you. More information about data protection and your rights can be found on the Club's Privacy Notice available on the Club website.
Payment Terms
1. Registration fee is a one-off fee for the Youth Programme as long as Standing Order payments are not stopped or interrupted during the season.
2. The annual fee may be paid as a one-off payment or in 8 equal instalments by standing order by the 25th day of the month of joining. Late registrations work on a pro-rated system. Please ask your team manager for details.
3. The Club will need a month's notice before a Standing Order can be cancelled.
4. Non-payment of a monthly instalment will result in the player's membership being liable to cancellation with immediate effect.
The player will then have to re-register should they wish to re-join at a later date.
5. Special cases should be brought to the Squad's Head Coach and then to the Head of the Youth Programme, whose decision will be final.

4. Customer Complaints
We endeavour to respond to all complaints or queries within five working days.
5. Events outside our control
We shall not be liable for delay or failure to perform any obligation under these terms & conditions if the delay or failure is caused by any circumstances beyond our reasonable control, including, but not limited to, acts of god, war, civil disorder or industrial dispute.
6. Licence
6.1 We grant you a licence to access the content, information and services contained within our website for personal use only.
6.2 This licence allows you to download and cache (using your browser) individual pages from our website.
6.3 This licence does not allow you to download and modify individual pages or substantial parts of our website nor to make our website available via an intranet, where our website or a substantial part of it is hosted locally on the intranet in question.

9. Limitations and Exclusions of Liability
